DOT Administrative Suspension Guidelines
 
 

1st Offense (over .10 BrAc Mandatory)

  1. Conact Iowa DOT to ensure eligibility for Temporary Restricted License (TRL) at 1-800-532-1121.
  2. Obtain SR-22 insurance from your auto insurance carrier.
  3. Fill out Iowa DOT form 430100
  4. Schedule an appointment for your Ignition Interlock installation. You can schedule online, or call 1-866-747-8278.
  5. Locate Iowa DOT driver's license issuing site.  www.IowaDOT.gov/mvd/ods/dlsite.htm
  6. After you have your IID installed take the following with you to the DOT.
    1. Ignition Interlock proof of installtion form. (We provide this when you are installed.)
    2. $243 to pay for your permit ($43) and civil fine ($200)
    3. Completed copy of form 430100 (TRL Application)
    4. Copy of proof of SR-22 insurance.
  7. Keep your TRLP on you at all times.  Especially when going to court, it can save you up to $625 in fines.
  8. Drive Safely!

2nd Offense

  1. Contact Iowa DOT to ensure eligibility for Temporary Restricted License (TRL) at 1-800-532-1121
  2. Make sure you have waited your 45 days hard suspension, or 90 if on a test refusal.
  3. Obtain SR-22 insurance from your auto insurance carrier.
  4. Fill out Iowa DOT form 430100
  5. Schedule an appointment with Smart Start of Iowa to have an Ignition Interlock installed.  Schedule online or call 1-866-747-8278.
  6. Locate Iowa DOT driver's license issuing site, www.IowaDOT.gov/mvd/ods/dlsite.htm
  7. After you have your Ignition Interlock installed take the following with you to the DOT.
    1. Ignition Interlock proof of installation form (Provided by authorized Smart Start of Iowa service center.)
    2. $243 to pay for your permit ($43) and civil fine ($200)

  8. Keep your TRL with you.
